A prosthodontist makes
artificial replacements for missing or damaged natural structures. This
can be as simple as making a crown for a damaged, decayed or malformed
tooth, or it could involve the design and fabrication of facial parts
for patients who have lost a nose or eye to cancer. Most of the time the
job of a prosthodontist is somewhere between these extremes. Patients normally see a prosthodontist when their dental restorative needs become more complex. Sometimes this will occur through years of wear and tear on the natural teeth. Other times it happens due to tooth decay or loss of supporting bone. Difficulties in matching shades or textures of the existing teeth may require specialty care. In addition to dental and facial restorations, prosthodontists are involved in the use of dental implants. Implants act as natural root substitutes and provide a level of security to the dental prosthesis that was previously difficult to achieve. This advancement provides an improvement in the predictability of treatment.

Post and Core (Pin tooth) Post and core is a procedure to rebuild a tooth in order to provide
proper support for a crown. The post will act like an anchor for the placement of crown. Post are usually recommended in case of inadequate support for a crown. A post is placed within the body of the root of a tooth that has already treated with root canal treatment. There are 2 different types of posts. In both cases, the result of anchorage for a crown is the same. The core is the part of the post that shows out in the mouth that help anchor a cap or crown. When The post and core is cemented into your tooth separate from the crown ,the crown (or cap) is then ready to be placed. Post and core are usually recommended when there is minimal support for a crown, but they are not always necessary. The use of a post will be determined on an individual basis based upon
support and structural requirements as well.
The first step is to remove some of the gutta percha that was used to seal one or more of the root canals in the tooth. Then, using a series of graduated rasps, the inside of the canals are enlarged and shaped to receive a specially made metal or metal-free "post" that exactly fits the hole made by the last rasp. These posts are usually pre-manufactured and come in many different shapes. The post is then cemented in the hole so that it is firmly embedded in the root of the tooth, and projects up above the level of the gums in order to help retain a filling that will be placed over it.
Metal-Free Post CosmoPost You can now restore endodontically
treated teeth using a completely metal-free system. The zirconium oxide
CosmoPost root canal posts permit
the fabrication of all-ceramic restorations in combination with the IPS
Empress Cosmo Ingots. All-ceramics are important to all people who care
about true-to-nature aesthetics. The CosmoPosts brighten the shade of the teeth and prevent greyish gingival margins. The root canal post is :
Fiber Post Fiber Posts are constructed from unidirectional, pre-tensed fibers bound in an epoxy resin matrix. This design produces a post with flexural strength exceeding that of metal posts but with a modulus of elasticity very close to that of dentin. The low modulus dissipates stress rather than transferring and concentrating stress in the residual root structure as with cast or prefabricated metal posts. Fiber Posts are intimately bonded to the tooth structure and core composite and their passive taper respects tooth morphology and conserves tooth structure.
ÆSTHETI-PLUS POST's universal
A2 dentin shade makes them ideal for use with all-ceramic restorations.
These
posts can be used with light-cured
or dual-cured adhesives
This consits of the following procedure. 1.Prepare root canal by removing & the gutta percha with Gates & Parapost drills to shape the canals to an appropriate size and depth. 2.The minimum length of the post is two-thirds the length of the root while maintaining 4- 5 mm of gutta-percha at root apex. 3.Impression root canal with silicone. 4.Submit the impression to laboratory for investing & casting in gold alloy. 5.Fix cast post and core with cementing material. 6.Once seated, prepared with diamond instruments to finalize preparation margins and blend the post & core with the tooth surface. 7.Take impression to fabricate crown or bridge as needed.
